PSA on the rise
I am on
Xtandi
and Lupron--(3 months). (I had taxetere in 2021) I get my PSA checked every three months right before lupron injection, so I did the routine blood work. I was suspisious that my PSA was probably on the rise as I have had some back pain in the last 4 weeks or so.

Enzalutamide (Xtandi): Start at half standard dose to extend efficacy time span?
Started
Xtandi
160 mg/di in April which pushed PSA back down to <0.1 but ALT liver enzyme up to 560 (ULN 59). This forced stopping
Xtandi
for 4 weeks until ALT was down to 69. Restarted
Xtandi
4 weeks ago on 80 mg/di. Today ALT 38 and PSA still <01.

Xtandi reduces effect of tamsulosin plus dutaseride ?
Dad has been on BPH medicine since 2011 to control symptoms of prostate enlargement - It contains tamsulosin and dutaseride Recently he has also added
xtandi
- enzalutamide I read online on drug interaction sites that enzalutamide can reduce efficacy of tamsulosin.

What a long strange road it's been.
Until the
Xtandi
, I managed a solo sex-life, a little weight gain and tiredness. The
Xtandi
OTH has obliterated all thought of sex, I have to make an appointment with myself just to think of having a wank, which still kind of works.

Xtandi vs Orgovyx
I'm also worried about the seriousness of the side effects of
Xtandi
, not to mention the cost ($3800 after ins). I've been doing some reading and found articles/web pages regarding the success of Orgovyx.

Sleep while taking Xtandi and Adt
I am taking
Xtandi
( aprox 16 months) Lupron, I had chemo. My latest PSA was less then <0.01. It has been there for over a year now. ( See my profile for my journey.) My main question is: when I go to bed at night, I never know if I am going to sleep 3 hours or 7 hours.

Decision Xtandi or radiation
The other option is replace Zytiga/Prednisone with
Xtandi
which I resist since Z/P is controlling all the other lesions and
Xtandi
can be used later if neecessary. OK, so its radiation or drug change. What do you guys think? I see MO Wednesday to make decision.

Xtandi and Orgovyx
I”m on Orgovyx and
Xtandi
of which things started to decline once we added
Xtandi
to the mix. My joints seem very arthritic.. From my feet to my hands. Getting out of bed is tough, but once I get going, the pain seems to subside a bit and flexibility improves.

Does triplet Therapy still work if you've failed Enza?
As most posters know the UK doesn't allow for other 'mides' once you've failed
Xtandi
(as I have done) But I am considering buying Abi from India to add to my Lupron and Docetaxel. Has anyone had success with Abi after
Xtandi
failure? Is the triplet therapy likely to work for me?

Xtandi and price tag
I started on Firmagon and my doctor ordered
Xtandi
. My question is …how can anyone afford
Xtandi
? My copay is 3300 dollars, more than my SS check. I’m checking with my MD to see if I can wait until Jan. ( to get better insurance) to begin

. Anyone have any input?

Clinical Trial Dallas UT Southwestern
We are adding a drug to
Xtandi
to treat solid tumors. I have been on and off
Xtandi
a few times. I am chemo naive. In the past had a very heavy tumor load and received 3 rounds of Lu-177 from Ishita Sen, Fortis health in Delhi. 18 months later 2 more rounds.

PSA still low but cancer progressing
The MO has said I need to stop
Xtandi
since it isn't working and move on to Docetaxel. I had a biopsy today of one of the groin (lymph) lesions which will be tested to check if Docetaxel is the right type of chemo to have.

If any fellow warriors have no further use of their supply, I'd be happy to purchase (or do a swap with

Prostate cancer morphed into NEPC
After 13 years of various therapies, the last few years included Lupron/Eligard, Zytiga,
Xtandi
. This kept the PSA low (around .2) and kept my MO happy. In 2021, I asked my MO if I should be getting a scan. He said with the PSA that low, nothing would show up.

Expericence with Xtandi
I had very good results for over a year with no side effects. It was part of a clinical study. My PSA is rising so time to move on to another drug. Good luck to you!
