At what psa is PCa suspected?
my 45 year old son has a
psa

Dumb Question about rising PSA after initial RT.
If you still have a prostate after RT treatment 7 years ago ( NO ADT), could a slowly rising
PSA
just be 'natural' as you age?

6 pluvicto treatments completed in January 24
Psa
decreased from 53 to 5.6 over 8 months. April bone scan shows progression. Mo says chemo again is next call. We are enjoying this time as still feeling good.

Anyone treated with enzalutamide +cabazitaxel?
PSA
never got below 3. Latest CT bone scan shows progression in a few areas. RO will zap the progressing areas locally. MO suggested cabazitaxel (1st choice) or enzalutamide (discouraged because cancer is aggressive). Any data/experience that combined enzalutamide and cabazitaxel would be better?

Is a false-positive biopsy possible?
As a result, for the last 13 years, I've had my
PSA
checked every six months. I typically run a very low
PSA
, historically .5 or .6. About 18 months ago I noticed that my
PSA
was rising, from .63, then .75, then .89, and then in September, it was .97.

PSA 6 months after RT
Ravi at ASCO 2023, regarding measuring the
PSA
at 6 months post radiation treatment, for all risk groups. He found that the 6-month post-RT
PSA
was highly prognostic of 5- and 10-year rates of metastasis free survival (MFS), Overall Survival (OS), and prostate cancer mortality (PCSM).

Should I Add Docetaxel to Lupron and Zytiga?
Perhaps, the reasoning is that if doublet therapy is effective at keeping one's
PSA
below .05, then the docetaxel wouldn't be effective. On the other hand, if Docetaxel can shrink bone mets, wouldn't it be helpful even though
PSA
is below .05?

PSA drop with decreasing Testosterone after Estrogen Patch Therapy
The answer is "Yes", and one can extrapolate the trend a short distance down to zero
PSA
at zero testosterone level. It's exciting to discover linear relationships from complex data sets! Bob

1 Year out Radiation treatment
PSA
undetectable three years, then back knocking at the door, (.17
PSA
rise 2022). BCR! PSMA scan clear. 35 IMRT Radiation treatments to surgery prostate bed, 6 months on Lupron. See my wonderful Radiation Onocolgist on Monday, Dr. Hung, at Oregon State Health Hospital on Monday.

Rising PSA with little shown on PSMA PET
My
PSA
came down to, IIRC, 0.42. I wrapped up chemo in Nov 2018 and that held well enough until the beginning of 2020, when he put me on Nubeqa. That took my
PSA
down to what I considered an impressive 0.04.

Radiation at low PSA levels
I saw an article or a feed somewhere for starting radiation for recurrence after RP that salvage radiation at around 0.05 level provides a higher potential for cure vs. waiting until it rises to 0.20+. Has anyone seen this article or statistic? Tx

FDA approves a new IND application of 225Ac-J591 (Actinium)
Specifically, among the patients who were evaluable for a prostate-specific antigen (
PSA
) change (n = 22), 21 patients (95%) in the study experienced a
PSA
decline. Of these, 14 (67%) patients experienced a decline of at least 50%, and 6 (27%) experienced a decline of at least 90%.

When is PARP introduced?
He is responding well so far as
PSA
is declining ( see profile for
PSA
) . I have seen some men are talking a PARP inhibitor and nubeqa and zoladex ( or ADT equivalent med's). Have been told by some we stay the course we are on till Castrate resistant than due to BRCA2 we would than try the PARP.

Best alternative PET for PSMA negative PCa
His
PSA
was undetectable but is now rising (3+) but PSMA-PET was negative, even in the prostate - implying PSMA non-avid cancer. What would be the best non-PSMA PET scan for him?

Next step options after lupron/zytiga failed
He also suggested talking with a radiation oncologist since it appears that those 2 spots may be the cause of my
PSA
rising and the disease progression. But I am worried about collateral damage from the radiation.

New Member saying Hello
My last
PSA
test was 9.64 in April, so my urologist recommended another biopsy, this time under sedation and (I think) ultrasound-guided. Of the 12 samples, one was (3+3) and two were (3+4). In some ways, it was a relief to finally know that there was cancer to explain the
PSA
rise.
