Question on MP MRI and Pirads score along with PSA
My father is 74 and he has been on
Dutasteride
and Tamsulosin for elevated PSA (6) for the past two years. His PSA jumped to 8 this year and then 11. The PSA never went down with Ditasteride.

5α-Reductase Inhibitors are Associated with Reduced Risk of SARS-COV-2 Infection
[1] Note: 5α-Reductase Inhibitors [5ARIs] - such as
Dutasteride
[Avodart] - inhibit the conversion of testosterone to dihydrotestosterone [DHT]. "SARS-CoV-2 has a disproportionately severe effect on men, suggesting that the androgen pathway plays a role in the disease.

PSADT was 10.3 months pre
dutasteride
and 24.8 months post
dutasteride
. Multivariant analysis showed a strong correlation between post
dutasteride
PSA kinetics and time to progression, with close to 50% relapse free at 10 years.

First Follow Up on MyTestosterone Trial
Intolerant/allergic to leuprolide so I did alternative ADT after BCR with bicalutamide 50mg plus
dutasteride
for 5 years before it failed. In 2019 I had Ga-68-PSMA scan showing only two PLNs in pelvis on left side. No activity elsewhere. So I did hemipelvis IMRT, 64 Gy with boost to the 2 nodes.

BAT, dutasteride, and my search for truth, Part 1 of 2
No answer that I trusted, one said no impact from
dutasteride
so continue it, but I do not think he really had thought about this..
Dutasteride
ok arguments (IMO): The following are excerpts from a much longer conversation.

dutasteride
based on Erbeta and when it comes and goes. Welcome to my rabbit hole.

Prostate cancer patients can benefit from 5-alpha-reductase inhibitor treatment: a meta-analysis
Although not a clinical trial, and subject to various weaknesses that the authors note, overall it seems to me a good argument for most of us to be taking
dutasteride
?

Anyone taking Avodart (Dutasteride) as your only form of ADT?
Snuffy Myers was prescribing
dutasteride
for many of his patients which I believe is a pretty good testimony for using the drug.

Adding Advanced AR drugs to ADT in non-Metastatic CRPCa Extends Overall Survival
And I am also curious about why DHT (5-a-reductase) inhibition via adding
dutasteride
or finasteride to anti-androgen regimens seem to be absent. Has this matter been definitively addressed?

5 alpha reductase inhibitors, DHT, and uPSA
A 3-week trial of
dutasteride
resulted in a 90% drop in DHT and a 50% drop of uPSA, both of which rose again over a several month period as the 5aRI was slowly cleared by the body.

RRP Oct2016, slowly rising uPSA over past year in addition to several other issues including hypogonadism, intermittent TRT, osteopenia, etc
Note that
Dutasteride
remains in body 4-6 months after being stopped. It has now been 6 months off the 5aRI. o In an attempt to address osteopenia and low fT, a two-month trial of Avena Sativa and DIM (Diindolylmethane) was tried but discontinued when E2 measured 209!!

PCa patients can benefit from 5-alpha-reductase Inhibitors.
{
Dutasteride
was approved by the FDA for BPH in November 2001. It has not been approved for PCa in the US. The doctor who wrote my script said 'good luck getting insurance to pay for it', but I have never had a problem.} "A total of 2,277 patients from 10 studies were included.
