Dilated cardiomyopathy & palpitations

I am a patient of dilated cardiomyopathy since 2018 & my EF is 34% stable for more than a year and almost asymptomatic. Prescribed with carvedilol 3.125 mg twice daily, spironolactone plus toresemide 5 mg, atorovastatin 10 mg, clopidegril 75mg daily. Before 10 months had a gout attack and is on febuxostat 40 mg daily. Before 3 months diagnosed with benign prostate hyperplasia and is on Tamsulosin & Dutasteride daily.

Of late for more than a month experiencing palpitations frequently and I can feel the contraction of my heart more fast.

The pulse rate ambulates between 80 to 50 during such times with oxygen saturation levels ranging between 97 to 100. However, quite frequently during such racing palpitations I can sense more mistimed beats & the oxymeter graph also becomes irregular during such mistimed beats.

The frequency and prolangation of the palpitations are gradually increasing everyday.

On regular medications, half salt and 1 litre fluid restriction with an average 2 to 2.5 km walk per day.

Had a review with my cardiologist before 15 days but didn't discuss about this since I thought it's under control but for the past few days it's more.

My electrolyte levels are as under.

Sodium 140 mg, Serum urea 45, potassium 3.9, creatinine 1.5, uric acid 6.0. HB 12.5 mg carbonate 25 & chloride 100.

A bit worried since the flutter has become more prominent, frequent & regular with duration increasing gradually.

Kindly tender your invaluable advice.

Hi and sorry to hear about your palpitations.With all that data you are very much aware of what your body is doing. Very impressed.

Something has changed and I would engage with your heart team to try and find out has changed and what needs to happen next.

Get a second opinion about what is going on. Something's not right.

Diagnosed as ventricular conducting dysfunction by my cardiologist & started ivabradine PR 5 mg once daily. Feeling better please. Thanks
