PSA hovering around 1 even after a year on ADT + Enza, should we insist on Triplet Therapy ?

Another vacation?
I was put on
degarelix
after 6 mos biological recurrence. A 2019 Auxumin scan showed small malignancy and lymph node involvement. I changed MO's in order to try intermittent ADT, hence the third vacation.

OVM-200 clinical trial
Back from a great holiday now and had my 4th non detectable PSA result last Friday so 11th
Degarelix
injection followed plus Apalutamide pick up. Online consult today with MO so have asked his thoughts on this vaccine trial.

Triplet Regimen vs Clinical Trial (Pluvicto)
I met with my oncologist at Sloan Kettering on March 23rd and received my first hormone treatment (
Degarelix
). My PSA has already dropped from 189 to 23.89, and I had my second hormone treatment, which was a shot of Lupron, on April 18th.

PSA rising - When to get a PSMA PET scan?
A year of
Degarelix
(Firmagon) + Apalutamide (Erleada) bought me more than 2 years of undetectable PSA. Now the PSA is climbing past 0.5 with a doubling time of about 5.5 months so a GA68 PSMA PET scan is probably the next move. I wonder how long to wait before getting the scan.

PSA increase in less than two months of ADT
He was given Firmagon (
Degarelix
240mg) on Dec.2. His PSA dropped to 500 on Dec.28. He's been much better since. January 2: Second dose of Firmagon 80mg. We finally got to see an oncologist two days ago, who recommends Zytiga and Chemo concurrently as soon as possible.

67 yo dad with stage 4 PC
He has started hormone therapy with
Degarelix
and already received his first round of chemo Docetaxel. He will be doing 5 more rounds of chemo every 2 weeks and then reviewed further to see how he is responding. His oncologist’s notes read - intent of treatment is palliative and non curative.

Seeing the < sign!
I know it’s all about stability but still after 7 months of
Degarelix
, 3 x Lu-177, 3 x Docetaxel, 20 VMAT rounds , 2 brachytherapy’s and 1 month of Apalutamide it was a good day! Feel like I should have ended that list with ‘and a partridge in a pear tree’! It’s been a mad 7 months!

Am I missing any options / pipeline developments that could allow me to stop HT?
I am on monthly
Degarelix
and daily Abiraterone. Clearly I love my PSA having the less than sign and want it to stay that way but am starting to be miserable with how much the HT is changing me. it’s not just the no libido which on its own is a QOL disaster but the energy and drive loss.

Lasting Degarelix reaction
Can there be tissue swelling around the injection site from
degarelix
5 plus weeks from the initial injection? This was 5 weeks ago with a Lupron injection last week. Only reaction to the Lupron so far was itching.

Testosterone “Flare” (Or Testosterone?) Timing to Optimize Salvage Radiation Therapy
Note that the LHRH antagonists
degarelix
and relugolix do not have the androgen flare. The article referenced is worth careful consideration by anyone here contemplating salvage radiation therapy to prostate, pelvic node fields and or oligometastatic SBRT.

Vmat rapid arc radiotherapy
Scans at chemo 1 showed ongoing faint prostatic activity after 3 x Lu-177 and whilst on

My primary treatment to the prostate was HIFU so apart from early Lu-177 I have had no radiotherapy to the prostate.

Micro-Mets Next Steps
I see my oncologist in a few days, and if Medicare will cover it, I am thinking I’ll be going on
degarelix
(Firmagon) supplemented by darolutamide (Nubeqa), both chosen because of past cardiac concerns (presumably heart and brain-safer than Lupron and Apalutamide).

For the chemo they would use a cold cap/ cold gloves/ cold socks to prevent hair loss and neuropathy Finally sorting a gene sequence on original biopsy tissue.
