I have SS and Small Fiber Neur. Within the last six weeks I have started with pain in my finger joints, primarily me thumb. It was very painful at the onset but is feeling better. Now other fingers are are hurting. I was initially tested for RA, but my Sed Rate was normal. Is this related to SS ?? Or could I also have developed RA ?? I see my Rheu. in a month. Thoughts ?

My neuropathy is in my hands as well as feet and legs. I get sharp pains shooting through my thumb and, to a lesser degree, the next 2 fingers. They call it carpal tunnel, though I have been told that designation can apply to neuropathic symptoms with a more generalizable cause such as SS. I have Sjögrens, but my neurologist is still looking for a cause of my neuropathy. I may see a hand specialist to help me figure out whether some hand pain comes from arthritis or tendonitis.

Do your joints swell, get hot and red? Or just painful? If swelling heat and pain it is likely arthritis and worth checking for RA. If no swelling redness and heat, then maybe arthralgia. For 'sublinical' arthritis, you could ask your primary GP for an ultrasound of your hand joints to screen for mild early arthritis. If no signs of synovitis (inflammation and thickening if the synovium lining of the joint) then arthritis or RA is unlikely. Doesn't mean you will never have it tho. If there is synovial thickening, it is evidence of arthritis in the absence of the usual signs like redness swelling etc.
If you can get this done before you see the Rheumy, and ensure the rheumy gets a copy of the results, that will help direct your treatment rather than the Rheumy guessing.
