I have to agree with both Elfindoe and also with Joolsg and others. It's a pretty pathetic survey but there is a comments box towards the end where you can type about your health issue and there's a promise that all comments will be reviewed by a neurologist. I certainly made good use of it (!) and suggest all other forum members do the same. See Joolsg post for the link.
