First Responders / Hospital Visits

Earlier last year I created this document so that if the emergency services turned up they would understand my wife and her requirements; It is in a on the door in a clear document folder as you enter the house.

It is also useful to take to hospital.

If anyone would like a copy in word format for you to edit with your own details then please send me your email address by internal chat.

I have downloaded a document from our local health trust called ‘My Care Passport ‘ I think most trusts have them it details everything about the patient.
