Is there anyone who could shine some light on my MRI results?

Lesion #1: Left mid gland posterior peripheral zone, somewhat rounded area of T2 hypointensity without circumscribed margins. Size: 1.2 x 0.7 cm DWI: ADC: Restriction. High b-value images: Bright signal DCE: No focal enhancement PI-RADS 3 No extraprostatic extension and the extracapsular fat is intact. The right seminal vesicle is unremarkable. The left seminal vesicle is absent. Bilateral bandlike T2 hypointensities consistent with background prostatitis. BPH with trabeculated appearance of the bladder. Tiny right bladder diverticulum.

Looks good. They will want to take some extra cores from the PIRADS 3 area, but the chronic prostatitis and BPH might fully explain your elevated PSA.
