SBRT-SIDE EFFECTS ???? (ACUTE-SERIOUS-LONG TERM????) or NONE

I have been emotionally worn out doing my final research and dealing with the ups and downs of this SOB..I have had opinions from some of the best Oncologists in the nation and all the info from these docs doesn't come close to this great group..

I am Gleason 4-3 PSA 6 (2 cores 55% and 10% , ) and my RO Mark Sholtz has recommended albeit unorthodox possibly AS and Other docs have Recommended SBRT.

I don't think I have the stomach to NOT treat this bastard somehow so I am again leaning towards SBRT w/ Spaceoar at UCLA with KInshan and Chang and Michael Steinberg.

I know this is redundant probably but could those who have had SBRT ( or any of it's forms ,cyberknife,truebeam,etc.) and just give a (quick post for all of us as to any and all of your side effects or no side effects!)

JWS, there are possible side effects with all treatments. I believe you have started other threads asking the same question about side effects.

One person’s side effects may or may not apply to your experience.

I am not sure what you expect different with the same question in multiple threads with essentially the same question about side effects?

You seem to feel uncomfortable with AS with 4+3, and seem inclined towards some form of SBRT.

It seems to me you have already decided what direction you want to go. How will someone telling you what kind of side effects they experience change that?

It shouldn’t. There is enough data out there on possible SBRT side effects, along with percentages, and remedies if you experience those side effects. The radiation oncologist should have told you the possible side effects, and if not, you should have asked him.

Mark Scholz isn't an RO, he's a urologic oncologist. He knows less about radiation therapy than I do (which isn't saying much), but that doesn't stop him from expressing opinions about it. When a patient hears opinions from another patient (like me), he knows to take it with a grain of salt. When a doctor expresses an erroneous opinion, it's harder for a patient to discount.

There is no competent institution in the world where a man with GS 4+3 is recommended to stay on active surveillance. OK, to be totally honest - I do know a couple of respectable doctors who question whether a GS4+3 if found only after an mpMRI and with a low percent (not 55% cancer!), it may still reflect insignificant cancer.

Forget Mark Scholz. Amar Kishan knows a LOT more. That's not to say I never disagree with Dr. Kishan. But when I do, he is usually right in the end. And it is always about fine points of research.

I had SBRT 12 years ago (Chris King, then at UCLA, was my RO). No lasting side effects (urinary, rectal or sexual) other than loss of ejaculate. It was before SpaceOAR, and I never had any rectal side effects other than some blood on my tissue, which lasted for about 2 weeks.

thanks very much for all the info... you are absolutely correct...it through me as well..I was all set for SBRT with Amar but when Mark voiced the possibility of AS ,the arena got confusing. I truly don't believe his intentions were malevolent, or thoroughly outrageous. He understood that I have a young wife 33 and we want to have kids, a relationship ...and considering all the imaging and screening available ..he was just putting it out there. He believed with appropriate screening we would be able to ramp up treatment if needed.

I am unable to live with a 4+3 every morning I wake up and worry each day that I may have a "rogue" cancer and it may get away from me...not so much to kill me but change the landscape of treatment to "palliative hormone" treatment if it metastasizes, which essentially compromises my relationship anyhow.. so I am back to SBRT.
