ALT = 107- NR =16-61

Globulin =2.5 - NR = 2.8- 4.4

Looks like liver damage is indicated, all was going so well...

MO suggests stopping Zytiga for 2 weeks and then retest CMP .

I just took a whole bunch of those on my own at UltaLabs with the rest of my FSA money now that I am retiring. Ldh, sed rate, fibrinogen, chromagranin-A, hepatic function battery with 10 tests in including those you mentioned and others. Bone-specific ALP. I don't think you can take them separately and read too much into it. They all sort of fit together. Those two are both high-but only mildly high the way i read the info and still, their ratio is about where you want it at less than 1. Actually I guess it is slightly over 1 but not by much at all.

I think someone on here said that if AST and ALT are 3x times normal that is the danger point

Think it depends on whether ALP was high. Those two are looked at together.

(European) Milk Thistle is a supplement to aid liver detox. Will not take your numbers back to normal range, but will provide some help into it.

My personal statistics before and after taking it, i.e. averages of 10 and 12 tests but, various labs with a bit different normal ranges:

AST: 27 before, 20 after

ALT: 32 before, 16 after

Are you taking many supplements?

My liver enzymes were increasing and my kidneys appeared close to failure according to my PCP. I stopped all supplements and within a few weeks, enzymes down and kidney 100% normal. I still take some supplements but I weeded out 75% of them.

I currently take SAM-e, baby aspirin, milk thistle, mineral sup. and a probiotic. I have been experimenting with melatonin searching for a tolerable dose.

what level causes kidney failure?

I do not know which one or ones caused it. Could have been impurities or heavy metals in some of the supplements. Even if I pinned it down to one supplement, they aren't controlled so who knows if, at some time in the future, another one would be just as bad or worse.

An aside: I saw a 3rd party lab analysis of psyllium husk (consumerlab). Something like 80% were above the FDA lead limit. Some were so high that if you had a tsp of psyllium husk you'd better not get a femtogram of lead from any other source. In other words, don't eat, don't drink, and slow down your breathing. Some of the things they test have NO active ingredients. Some are very clean though and certain categories are better than others. Seems to be divided by natural: expect impurities and suspect amounts. Synthetic: usually good.

I have PSA labs every 3 months.But since starting Abiraterone I have monthly labs to monitor liver.

Abi has detrimental liver side effects for some men apparently occurs often enough that the monthly liver lab work is SOC.

Well lets hope your Dr. has an innocuous explanation.Still if it is related to Abi--- Nubequa can be used instead and just as effective. I think.

Zytiga also increased my ALT/AST. Your MO is correct that Zytiga should be stopped. After my levels returned to normal within a month or so, we rechallenged Zytiga at 1/2 dose. Liver levels rose again. Stopped drug permanently. Your MO sounds like a good one, I've spoken to numerous men who's MD, often a urologist, did not monitor blood work every 2 weeks initially as the drug manufacturer states is necessary.

We are now on a every 2 week schedule until this resolves.

Your MO is correct in checking you often. There are alternatives to Zytiga. There are no good alternatives to a damaged liver.

Is this the first time these have been measure?There can be other issues that raise these enzymes as well.

Non alcoholic liver disease has become very prevalent these days with 30-35% incidence.

There can always be more than one thing going on.

two weeks ago AST and ALT were just slightly above normal

I had the same problem when I started on Zytiga. I was on Lupron as well. We stopped the Zytiga and all my supplements, though there was nothing I was taking that should have been a problem. Things started to improve after a month. At three months I started back on a half dose of Zytiga. After 2 months of that the Zytiga dose was back to normal and I restarted my supplements. All was well from that point on. Good luck.

In the beginning my bilirubin, ast and alt bounced all over the place. I was freaking out thinking that kidney failure would get me before the f**ing cancer. After about 6 months my results stabilized. Sometimes they are a little higher, and sometimes they are lower, but they stay withing "acceptable" range. Alsoe, even when they have been above range for a couple of months, my MO has advised waiting for a longer trend before panicking.
