New and What happens next?

I am new here, and I looking for opinions, as I am about to have a second biopsy, nervous and wondering what to expect. Just want to get results and have everything under control.

After a very resent, Blood, very by an immediate MRI, and now to do an biopsy after just a few days. My latest PSA is 21.2, free PSA 4.8%, Velocity is high... 4 months ago PSA was 14.4. MRI showing a possible PI-RAD 3. I have no other symptoms.

Quick history

March 2021 sent to a urologist as my PSA was 11.02

Found out a past doctor did not flag this the records I found had a PSA of 0.8 in 2016, then 10.4 in 2019.

Urologist PSA below.

April 21 - 8.57 (after a course of antibiotics)

May 21 - 13.1 (Biopsy Clear)

June 21 MRI Clear

Sept 21 - 12.4

Nov 21 - 8.85

Feb 22 - 18.9

Mar 22 - 14.4

Mar 22 - 21.2, Free PSA 4.8%

You may have prostatitis. Prostatitis often causes sudden spikes in PSA and the see-saw pattern you observed. It is usually unresponsive to antibiotics, shows no pathogens, and may be asymptomatic = hard to diagnose.

Next time you have a PSA, ask for a Prostate Health Index (PHI) test instead. It is much more specific to prostate cancer than PSA or free PSA (it includes both). There is also a test on your biopsy cores you can do called Confirm MDx. It is not PSA based and can give a good indication of whether your negative was a true negative.

I will most certainly ask for a PHI,. Also hopefully after the biopsy I will get the MDx, I will ask.

I have my biopsy in the morning, but it will be a long two weeks waiting for the results.

Hi Acesce12,

I am about to be 62. I had a second biopsy Saturday, the other was a year ago. Get results in about two weeks.

I am for sure hoping it comes back negative . The MRI showed a possible Area of concern, but this was not clear, as I have two false hips, hence the biopsy.

Fingers crossed tight now waiting for the result.
