I was diagnosed with 3+4 favorable intermediate risk PCa, with a lesion being 7mm. PSA prior to treatment was 4.9. I chose SBRT 6 months ago and was treated on a Cyberknife machine.
3 month PSA = 2.89
6 month PSA = 2.60
I was expecting a more dramatic decline in the PSA and now have quite a bit of angst. I understand it can take a long time (even 3+ years) to reach nadir, but the slowing reduction in PSA makes it appear less likely mine is going to reach the "goal" of 0.5.
What are the indications of a PSA nadir of 2.0 or 1.0? Does it make it more likely I am going to have a reoccurrence?
Thanks for any insights.

Let me give you my experience…4 months s/p proton therapy April 2018 PSA 1.37 then every 4 months increasing right up to 6.5ish which same as where I was diagnosed. Then steady decline to Nov 2021 .83 and hopefully continuing down some more. Needless to say, I’ve had my share of stress over the last 3.5 years. Everybody is different but nobody told me that it wouldn’t be a nice linear decline.
Six months after HDR-BT my PSA had fallen from 6.3 to .84 - I was thrilled. Six months later and it's climbed back up to 1.72. Next test the beginning of Jan. when I expect it will be even higher. Doc says all is okay unless it hits 3.0 and then maybe a scan. Should settle down in the next 6 months or so. No worries my friend!
