Orchiectomy vs Intermittent ADT

Why would a man in his early to mid 70's who has already said bye bye to his sex life due to age, RP and SRT choose (what appears to me) the rollercoaster ride of intermittent ADT vs a simple orchiectomy with the potential of controlled hormone replacement in the future if the opportunity presents itself ? I know this question is somewhat "over simplified" but I hope you can get my drift. I'm not dead set on this but just trying evaluate my options' pros and cons.

First, intermittent ADT is in my opinion, risky. Better to go with continuous ADT.

Next, if you have had SRT and RP, I would not go with an orchiectomy.

I am curious also. I had a prosectemy 14 months ago, doing well, but a slow rise in PSA, now .20. Still dealing with ED, desire but no performance. Thoughts on what steps to take if any, I am in mid 70's. Thanks.

I would think radiation therapy would be the next step. Your PSA is still low but if it continues rise take action. You doctor may prescribe Lupron or another ADT drug before your radiation treatment begin.

Am pretty sure that in third world countries the poor guys get an orch right away? Living on high price drugs is a first world issue .

He is in his mid 70's. I would think having another operation might not be advisable. ADT treatments could provide quality life for a number of years.

Thanks for the return. I understand your thinking and I agree with continuous ADT if one can and does adjust to the side effects, if any. Of course overall health in mid 70’s should be taken into consideration..

I was myself faced with shots for life supposedly at 53 yrs old . I did the chop after 18 months Lupron . Doc said it was ok . I’m still on a pill adt stopping adrenal production t ?

What, if any, side effects have you experienced with the pill ADT, and if I may ask, what is the pill ADT you are taking. Thanks for your help/info!!

I’m taking Tak-700 . IMO the side effects are one reason it failed . Sarcopenia nausea and many more . It was bad with Lupron for me . Most men couldn’t take it and dropped off quickly . Not me .Im a glutton for punishment . It as bad with Lupron . It’s a defunct test drug that only a few are still on ..I’m the only one on this cite ever to use it . . It is supposed to stop the adrenal production of t . I’ve had 3 t for over six years . No t is a disability in my broken book .when I was told that would kill me I said “ off with the juevos” ! Doc had me wait 18 months . That was 4 yrs ago . No shots . I’m in osteopenia and my joints are going painfully south . No pc or Psa just living with the after effects of a few yrs of hell early on .
