A friend told me about this site and I thought I would check it out . 60 years old psa creeping up 4.1 and had a truss biopsy 11 neg 1 high pin that was in January, June psa 4.1 and urologist told me Mri in October and it showed 3 pirad and a 4 pirad now I have appointment Monday to talk to urologist and see about bore biopsy or MRI targeted not sure where I will have to go for that . I live in Panama City beach and none of the hospitals have it here so hopefully the urologist can lead me in the right direction sorry about the rambling

It seems that MRIs are a topic today.
The mp 3.0T MRI is the best. If necessary an in bore, real time, biopsy can be done. One of the best in the country is in Atlanta, Georgia.

I have never spoken to this doctor but he is one of two whose careers I have followed that I would choose. They are conservative when biopsying and very accurate. They have excellent reviews from former patients.
The second doctor is Dr. Karamanian in Texas.

If you're going to have a needle biopsy have the doc knock you out. They are not very pleasant. Most urologist do not want to put you under sedation since it requires an anesthesiologist and generally in a hospital setting, not in his/her office. It's up to you.

I am 73 y.o. with a G(4+3=7) Grade 3 Unfavourable Intermediate Risk.
I had 12 cores biopsies under local anaesthesia, not the gel but Lidocaine 2 % injections.
I did not feel ANY pain but felt 12 pushes, period.
If you have a urologist that have done thousands of biopsies, you should not fell any pain.
6 positive cores on the right lobe but on my planning VMAT-RT Cat-Scan they found many tumors in both lobes.
And now PSA = 0.03 µg/L and T = <5.7684 ng/dL. Just the way my RO and my Urologist want.
Unfortunately I later found out my urologist is legally blind....

Well I've had two cystoscopes by two different urologist. But just a routine exam no problem with burning sensation when urinating. Both of them were a breeze and "no biggie" I read where T A is against it and I would listen to him (he knows his stuff). Did doc check you for a UTI? Do you have a high temperature? I wish I could advise you more...and I do hope it's nothing serious. Keep us informed....

I had 2 urine analysis and cultures that both were negative. I knew that the test is easy and does not hurt.
The thing that worried both my RO and my urologist is the fact that I have that problem for over 2 months.
So they want a cystoscopy to see if it is only irritation or the VMAT-RT has damaged the penine bulb which was irradiated for about 5 cc.
The urologist is not planning any biopsies. Just IN, look prostatic sphincter and the bladder sphincter, then OUT.
Well it sounds like and "in and out" procedure that's anything that can't be resolved quickly...Bring a stop watch with you to impress upon them that they complete the test quickly... You may also want to do loud "TICK TOCK" sounds at the same time.
(I hate all this shit we have to go through)...

I talked to my Urologist and he told me that 1st the cystoscopy was needed to check the 2 sphincters and see if there is inflamation or irritation or scarring in the urethra, 2nd he NEVER had any case of patients getting permannent damages to the urethra.
And 3rd, I checked an my urologist is NOT blind and does not have a white cane neither a dog, So I am safe for that.
