Inability to ejaculate...

9 months after completing one lipron injection, external radiation and brachy, i have returned to low end of normal testosterone, good libido/desire, 75% erection strength, but at point of ejaculation, nothing happens and lose sensation and erection. Rare but sometimes pain. Had some issues removing brachy device during treatment and 6 weeks after scrotum turmed black from blood, but cleared up within two weeks... Any thoughts? (61 yrs old)

They should have told you that 95% of men having radiation and 100% of men having surgery permanently lose semen.

Nobody ever told me that at imrt! But Why wouldnโ€™t this be so . ? After orchiectomy , definitely no soldiers .

It's not sperm, it's semen. Semen is produced by the prostate and seminal vesicles. Sperm is produced in the testicles.

If you had RP or RT to the prostate, you don't have semen.

I know a few guys who still ejaculate at orgasm after RT. My RO believes the RT causes scar tissue in the ducts of the prostate, which impedes the movement of fluid. Eventually the semen glands stop producing it.

Will the scar tissue of the ducts of the prostate ever heal and recover in time. I have plenty of semen cummings out but no sperm I think? Tomorrow I see my Urologist to see if sperm is present when I ejaculate into a cup. Hopefully there will be a trace of sperm in my ejaculate.

I don't care. Was just saying that I WAS told about the (obvious) no sperm, but was specifically told I WOULD still ejaculate the remaining contents of seminal fluid.. which is not true.

Actually told me the opposite.... Lose sperm, so seminal fluid decreased in volume, but would still have it....

Seminal vesicles and prostate are removed in a prostatectomy - nothing left to make semen. Sperm is made in the testicles, so they are not affected by RP or RT. There have been case studies were sperm has been extracted for insemination post treatment - sounds painful.

It is my understanding that although there is no sperm produced, The other fluids that make up the ejaculate should still function and i shoukd still have an ejaculatory response.....

You can and probably can have an orgasm but since the seminal vessicles are destroyed there aint no way you can ecaulate liquid. There are some are cases where guys do, but into their bladder and that is not good. The orgasm post radiation is different and it takes some getting used to since we associate the orgasm so much with ejaculation. At least that that is the way it was explained to me and is my experience. Also from STL.

Just read the article suggested by fluffyfur above, which confirms what you say... although it looks like I'm in the unlucky 16% who have the problem immediately, rather than after passage of time.... Wish somebody had told be about this up front, rather than having to experience it.... Thanks for your input.

I had 'retrograde' ejaculation after TURP surgery for an enlarged prostate, years before my prostate cancer diagnosis. After HIFU ablation of my prostate (tumors) I don't even have that anymore. No big deal. I had no plans to father any more children before all this anyway.

The prostate gland, under numerous treatment scenarios, will no longer function 'normally' post treatment.

You are kicking the snot out of it, so you should expect that things won't be the same.

Some communication from the medical professionals about what to expect was not adequately covered or reviewed.

I suggest you get more consultation and / or supplemental reading, so you understand what is actually happening to you and what might be the resultants / expectations from the treatment(s).

That's why the get paid the BIG bucks ....

I have an appointment for a follow up next week, and will get more info from a nurse practioner, but it sounds like I was just not informed of this reality.... Not happy about it, to say the least, but at this point am just interested in finding ways to address the issue. Can get used to the lack of ejaculation, but the "at the brink" and then just gone feeling, without orgasm/release, is something that I want to delve into fixing, if at all possible

The orgasms are different once the treatments have been initiated. They are called 'dry' and the intensity may also be affected. That doesn't mean it's over 4 U sexually, you just need to adapt because you can still enjoy sex - Information makes a big difference in accepting the changes.

Been there, done that.

It isn't an intensity issue. Its literally like going from being on the peak of a cliff, straining against a harness that is barely keeping you tethered, to suddenly being in your living room, sitting on the couch, watching an amateur golfer sink a one foot put.

In my case it's different because I had radical prostatectomy. I have an inability to ejaculate too, I feel the orgasm and the muscular contractions but the fact of no longer having semen which spurts takes away a lot of pleasure and sensation, it is very different anyway

I had focal HIFU, and obviously carry scar tissue as a result. I am trialling serrapeptase to try to reduce internal scarring, blockages, stricture, whatever ails. I can tell you so far that it is amazing stuff for the reduction of any inflammation anywhere, amazing pain reduction in my lower back from damage long ago, and amazing reduction in long term knee pain. It should help clear necrotic tissue and scar tissue and I wish I had tried it earlier. Cheers

What brand of Sereap do you use I have been looking at various brands and although that is how they are labeled. when you look at the ingredients it's actually mostly other stuff... I'm assuming you will would wanted to be as close to 100% as possible right?

Hi. I'm not a brand nerd. I look for the cheapest I can get at the highest active content. eBay is my friend... You could also look at India and China for pure serrapeptase powder in bulk. I seem to be unblocked, against the odds and against expectation. A caveat.... MMPs (metric metallo proteases), of which serrapeptase is an example, are implicated in metastatic spread. MMP13 I believe has been identified as Bad. I have contacted many researchers and professors for a view on serrapeptase in this regard, but no-one knows.... The opposing theories are that SP can unmask cancer to allow attack by the immune system, or it can remove inert fibrin matrix to provide a comfortable spot for cancer cells to hide in. I wish someone knew the answer. Cheers

Thanks for the info.... Didnt know about the met spread. Started taking it as an ad hoc covid 19 preventative based on an article out of Japan, rather than the ejaculatory issue, which is why your post intrigued. Sounds like I may need to do some digging...
