your midwife should have filled a form in on your booking appt which you should have signed, you then receive a card through the post stating your excemption.
I literally found it after I posted the question
I got my form from the gp, took about three weeks for my card to arrive
I went directly to be dentist and they sorted this out. I went from being a private patient to an nhs patient and I signed some form so I am assuming I'm now free until the baby is one.
