Are you in the UK? Seems it might be linked in some way to gp info? I have been able to down load it but not sure if that will be help if added here?

Not sure but I think sent by a PMR ambassador who also only had these two pages.

I'm the only PMR ambassador as far as I know and it wasn't me. However - without knowing the SOURCE, no-one can identify what it is.

Thank you everyone who replied. The article for which I had missing pages related to entitlement for this test via the GP, but I believe I now have enough info as I was successful in following the link you gave. How useful this forum is for us all.
