My periodontist will be removing a piece of tooth left in from a dental implant. He suggested I increase my prednisone dosage from 6.5 mg to 15 mg for a few days prior to my appointment.

Does this make since? My tapering has been proceeding without problems except fatigue.

Personally l would not go against your Periodontists advice, at 6.5mg your adrenal glands are still not working as they should be as indicated by the fatigue. It may be difficult to remove the broken piece of tooth so may cause stress on your body which you will be unable to deal with.

Agree with MrsNails , you should comply with medical advice, unless there’s a very good reason not to….you could check with Rheumy if you are unsure.

He is saying it as part of Sick Day Rules - he obviously views it as a potentially stressful procedure though I'm not sure why he's saying to increase it so far in advance. The day of the procedure and a couple of days after would be more usual.

It isn't disrupting your reduction - you can take a higher dose for up to 7-10 days and drop back to the normal dose without any problem, it is just the same as a short course of steroids for a chest infection or asthma, the difference is the baseline is higher as you are on pred so you drop back to that rather than to zero.

Thanks to all of you for your replies. I apologize for not responding sooner but I was out of cell range for several hours. Base on your feedback I will start the 15 mg dosage 2 days before the procedure.

Hello! I recently had an extraction of a big tooth with 3 roots, and all went well.(I was on 4mg then and did not alter dose). The surgeon gave me painkillers beforehand, and I can honestly say it was a straightforward procedure. He advised me to use warm salty mouthwashes from the next day, and I think this really helped with the healing. Good luck and best wishes!

Seems a good idea to me. However calm you may be feeling about it now, a few extra mg will help to deal with any stresses, physical or psychological which may be triggered by the extractions.

I've been in the same situation this week. Had dental extraction on Sunday. I was on 1mg pred and didn't increase my dose. I had a debilitating headache and fatigue for three days, and on Wednesday and Thursday upped my pred to 10mg. I felt much better by Wednesday evening, and this morning went back down to 1mg. After a morning's gardening so far so good. I would really recommend you to increase your pred dose as PMRpro says, per the Sick Day Rules, for the day of the procedure and a couple of days afterwards, and then drop back down straightaway to your usual dose. It won't scupper your reduction plan. Good Luck!

Thanks. This blog is incredible. You get information on issues on both sides of an issue based on individual experience. I have not found any other source for information on PMR that comes close to what we have here.

Update: All went well with the minor oral surgery. Back down to 6.5 mg. Thanks for all the good advice!
