Have you had hip X-rayed? That will tell you if arthritic and needs replacing.
If not, then could be bursitis. That won’t show on X-ray as it’s soft tissue problem not joint issue.
A lot of the pain of pmr is a bursitis like pain. I have had pain so bad in the hip and groin I was sure I was going to need hip replacement and have gone to get it X-rayed only to find out nothing shows up. That is great, but I am left with the pain. After it while it goes away, only to move on to somewhere else in the body. I find ice really helps when it sets in.

Well osteoporosis showed up alright, nobody wants to blame the steroids, however how did,I not have hip pain before taking this medication?
I think hip pain can also be caused by referred pain from the spine if a nerve is being compressed.

That is very true. Today I was in surgery and had steroid shots in my low back for pinched nerves. I am anxious to see if that helps the hips at all. Fingers crossed!

Do you know your t-score? That gives you an idea how serious it is. And once your steroid dose is very low, perhaps to 2 or 3 mg, any impact on the bones will be negligible. In fact I think it is really in the first few months of being on pred that the greatest impact occurs.
OA is relatively simple to identify with imaging. If it isn't OA, then it probably becomes a clinical decision. There are typical symptoms - which the local orthopods, my GP and my rheumy are all able to recognise.

Thank you for reply I am down to 7,5mg and since the physio gave me some excercises which I didn't want (she did give me a muscle going over which I did want) my right hip has been playing up. Due to age I suspect. I had a lumber MIR scan recently and wonder if it would have shown up on that.
No. You need an x-ray or MRI to identify OA, you have to look at the state of the joint. Dexascans just look at the bone.
Don't know if I have sent this. Physio gave me some excercises which I didn't want (also muscle going over which I did want) had trouble with my right hip which has played up in the past. Do you think my latest lower back MRI Scan would show up the OA?
The hip pain is totally disabling with OA requiring hip replacement, at least that is NHS position. I was having to use a wheelchair. Also, not an operation to be taken lightly. It has dominated my situation for over a year, but then I have required 2 hip replacements.
Today, is a red letter day. I have managed to get into the swimming pool to exercise. Valerie
Osteoporosis itself is not usually painful. Other things can happen, like weakened muscles or such things as bursitis and sciatica, which could be causing pain at the same time as the bones have been thinning. And people who experience compression fractures of the spine will feel pain because of nerves being impacted. I didn't think bones themselves had nerve endings which could transmit pain but someone told me I was wrong about that. When I had a broken leg any pain I felt was from the surrounding tissues not from the bone itself.
