I know people use medical alert bracelets, notes in wallets etc.

Currently I'm using Medical ID (free) V 6.5.0 on my Android phone.

It's just been updated to resolve an issue with Android 8 and is working fine.

From you phones lock screen you can double tap on the app and open up the details you have registered including ICE contacts.
