I have worked in the medical field all my life and spent over 30 years translating science and medicine texts from German to English - you do need a lot of background knowledge. And I've been learning about PMR as a specialist subject for the last 9 years now (that's scary!) and have lived with its effects for 14 (even scarier) ...
