It may just be the return of old arthritic pain.

Are you feeling very tired, craving salt, feeling cold?

I just ask because at your dosage the Adrenal Function can be problematic and actual failure of the Adrenals can be heralded by excruciating low back pain ( kidneys). I know you chaps tend to understate your symptoms. Now you'll tell me you're female!

You can ask to be referred to Hospital to have your Adrenals tested via a Synacthen Test ( a blood test following an injection) if you think it might be that.

Or perhaps you've overdone things. It doesn't sound like a flare. You've done well!

How fast have you reduced from 8mg? And where is the pain?

I would have said exactly same as SJ so won't repeat it.

Then stop pushing it and stay where you feel most comfortable. This is a low dose and will do little longterm harm:

If you don't want to go up again to be pain-free that is up to you. But if you leave things when the dose is slightly too low, eventually you will be back where you started with a lot more pain as the inflammation mounts up. Then you will have the choice of the pain or a much higher dose.

If 4 or 5mg manages your symptoms then I would take it - after 13 years the likelihood of you getting off pred altogether is small so you might as well take enough to be comfortable.

If it goes down your leg the most likely thing is sciatic pain - which can be due to spasmed muscles in your back - myofascial pain syndrome or piriformis syndrome are the most usual causes.
