Hi not 100% sure but I think it's the forms you filled in before your medical assessment by ATOS if your still not sure phone DWP regards neata

Well I am certainly finding out certain things via this site in case I ever need to apply for any form of ESA which is apparently the new PIP.

You got me intrigued and all I can find out it that it is apparently the file that was put together on you and as Neata stated it contains all the paperwork you and probably a doctor have submitted as well as their own internal paperwork.

tattybear - I am trying to find out what the ESA55 is.

ESA 50 is the form you fill in to apply for ESA and is where you tell them all about how your illness affects your activities.

Peridot: PIP is the new DLA. ESA is the new Incapacity Benefit. After april this year, (i think) Incapacity Benefit was phased out and claimants were 'migrated' to ESA.

DLA has just been changed to PIP (Personal Independence Payment) within the past month. It replaces DLA which is being phased out for existing claimants. Anyone making a new claim (or I think reporting a change of circumstances) now applies for PIP.

For people who are struggling with memory and concentration (like me!) all these changes are a NIGHTMARE!!
