Scalloped tongue just appeared had 4 loading doses

Sorry I keep posting my partner is just so down with all symptoms. Hes had 4 loading doses due 5th one today and hes just developed a new symptom a scalloped tongue. Does this mean its not working. Any advice greatly appreciated

Make sure folic acid taken now if level was on the low side.A multivit usually has 200-400mcg in it.

Symptoms do appear or get worse in loading phase at times .

I developed tinnitus.

Also got heart palpitations.

Waking up nerves not nice at times .

Like rebooting.

A blood test 3- 4 months after loading not to include b12 can be useful to know how iron,ferritin folate,vit D

Good to see you use the expression "rebooting". That's exactly how I see the the process of recovery. We are rebooting the whole physiological system after years of gradual decline. And rebooting takes a lot of energy, attention and care.

They didn't test vitamin d when I asked why they said its not usual with the symptoms he presents. Iv got the ferritin and folate levels, we were told folate levels good but im struggling to intemperate them

Ferritin is set at avery low point of withing range I had symptoms at the level of 29ug/L

Range ( 11-307) it's only a part of the iron picture .

Can rise if inflammation in the body.

I 2as told to keep it between 50-70 ug/ L

As had restless legs snd hair loss and heart palpitations with shakiness .

It's very difficult to know what s what in the early stages with so many symptoms present.

As for vitamin D

I was given loading doses whe the level was 19

Prior to getting ill with b12 def.

A sign in Hindsight all was not well.

On supplements of 2-3000iu daily my level is 78nmol/l

Satisfactory on my results states 65-250nmol/ l

All labs differ.

The in range is usually in brackets on the right .

Some results atd 8n % I don't understand those.

Folate has s vast range too.

My result after supplements just stated above 26

Was 5 so never that low on folate .

Thd labs often tesg folate with b12 .

Excellent at the start.

Not so good when you just want folate measured.

Anything you are unsure of ask

Sometimes if the computer says normal results arnt looked at in detail unless you ask.

I appreciate this is very difficult unless a consult booked.

From what i understand only b12 low on that sheet Interesting ferritin bottom figure higher than it used to be. .

Possible causes of b12 deficiency given.

Intrinsic factor mentioned

IFAB test not mentioned being done

That would show 50% of those with PA.

Usual test used.

Parietal cell antybody test looks like has been done .

Not usually used to diagnose PA .

As can be positive for other things .

If my memory correct if positive PA suspected. 80%

If negative 20% coukd still have PA .

There is not a definitive PA test.

I can't read the other sheet and probably above my understanding.

From what I can see it's all within range.

Result given .

In brackets range given .

Like I said ask for anything to be explained as is is looked at all together especially the iron by a trained eye.

Megobolastic anaemia looked for

Total iron ect .

Bloods are normally repeated 3-4 months .

Ask for vit D then.

Others might know more .

Its outrageous for them to describe a serum B12 level of 123 ng/L as "Borderline Vitamin B12". The "borderline" is 180, according to their own ranges. Borderline might arguably be 160-200. The measured level of 123 is clearly and quite significantly below range. Can they not read whats in front of them? I'm baffled at the stupidity.

If you're b12 deficient and treated with injections the body starts using not only the b12 but also other nutrients needed in the process. So levels of available folate and iron may actually sink because of the b12 treatment, at least temporarily. I've read som recommendations that because of this ferritin and folate should ideally be retested after treatment has started, but I think it's rarely done.

So when the body starts repairing itself you may get and increased need for nutrition.

Here's an article that states that b12 deficiency can mask iron deficiency, because as long as b12 is low the body can't make normal use of the iron to make red blood cells.
