I'll bet it's another postcode lottery. If you were at Hollerton University Hospital then they do an Active B12 test automatically if serum B12 is less than 350 ng/L homerton.nhs.uk/our-service...

This document for New Cross Hospital lists the places they need to send samples for various tests. royalwolverhampton.nhs.uk/E... It includes an entry for Methylmalonic acid and holotranscobalamin - St Thomas' Hospital, London.

If they have an entry in their Referral Test Directory Book for the Active B12 test then they must expect somebody to want to send off for such a test.
