Hello,

I’ll like to know more about the “time-off” phenomenon. I am especially interested in scientific articles. Who can help me?

Kindly Pieter

I presume you mean “off-time”. This refers to the time that your levodopa blood concentration is below the concentration necessary to eliminate your PD motor symptoms... (ie, the time your are undermedicated)...

Can you explain a little more what you are interested in finding out?

I have PD myself and one of my daughters works as RN in a care home with several PD patients. She wrote me: “Please ask for an evidence based article wich explains the off period. And how nurses and docters deal with it.

We are doing a research in a care home which shows thst most patients have too many off periods”

In my experience the 'off-time' varies from day to day.

That said, I also believe that the gut activity & inactivity plays a key role, and a varied diet with fruit, green vegetable fibre; plus staying mobile with walking exercise helps to reduce occurrences.

Your daughter should be able to find material using google. This came up for me. Its old but Proff Lees is a well known neuro. I havent checked it.

Hik:

Every PD patient (and their doctor) should "read and study" Dr. Lees's work in its entirety. Somewhat dated (1989) and quite a bit to digest, but at least worth it in the long run.

Here are a few more links. You will probably find info on PUK site too. They have a campaign Get Meds on Time. It is aimed at care homes and hospitals to ensure the residents get their meds on time otherwise they go off and are unable to function.

In the PD support group we go to for husbands PD, off time is the wearing off of med. effects before next dose is due....for instance my hubby takes Sinemet at bedtime so his off time would be in the eve I guess. He is sleeping in front of T.V. Most all day so I don't know if that's off or not. We're mid 70's and not motivated at all tho he is in a Control group ( do what he wants or nothing)of an exercise study so he is in a boxing class twice a week which isn't part of the study and not doing much to change things. His med is the same, tho I can't get much info out of him after Drs. Visits. He has stiffness upon getting out of sitting position, tremors mostly in eve, mild hallucinations. Also double vision in one eye which bothers him the most even after trying corrective surgery. This has been after cataract surgery on both eyes. I think someone goofed on his right eye. M.A.
