Sorry to hear you have EPI. I was diagnosed a year ago, also given no advice. And it’s been difficult. I live in the US in a smaller city where there seems to be limited help for people with EPI. My best advice is to go to the Mission Cure website for information and advice. They have educational videos plus videos by 3 of the top doctors in the field, one British, one Canadian and one from the US. What I’ve read is that a health care team for a person with EPI needs to include a pancreatic specialist doctor, a dietitian, and mental health support. Everyone is different, of course but generally eating smaller meals more often, and limiting animal fat should help. There is also a proper way to take the replacement enzymes through each meal. A healthy well rounded diet and walking is helpful as well.
Here is my opinion - no special diet (unless like me you are coeliac) I will now add a reply I gave yesterday to someone else entering the PERT phase of their life - most of which is relevant to your enquiry. NB Creon25000 and Nutrizyme 22000 are broadly equivalent....

This insight came alongside the 2023 PERT shortage but my newfound revelations woke me up to the need to attend to my biome and I started an intensive course of probiotic capsules (Weightworld Bio Cultures Complex Probiotics & Prebiotics).
Within a few weeks my toilet habits were transformed from up to 15 visits a day (Often explosively and uncontrollable) to just 1-3 per day of proper poo! And the incessant farting has all but stopped!
I do not know which of the two treatments have had the most effect so will persist with both (although I now have reduced the bio cultures from 3 to 2 a day and have a fairly reliable supply of PERT (8-10 capsules of Nutrizyme/Creon per day)
Perhaps you can modify your PERT dosage but I would certainly urge you to consider the probiotic/prebiotic approach.
Never having experienced stomach pain, bloating and had the dreaded piles sorted I can only sympathise,."

Hello, Creon has an "Ambassador" program that pairs you with a professional and they should be able to guide you. I kept a food diary, dosage and poo journal for two + weeks. I'm 105 pounds and need two pills with breakfast, two with lunch and three with dinner. One pill with each snack. High fat foods blew through this regiment but lower, healthy fat foods were OK for me to digest. Everyone is different, I found it is not based on weight but how bad your EPI is.
