Can it be ITBS if it doesn't hurt when running downhill?

Lately been getting pain on the outside of my knee, it’s dull and aching and usually comes on 4km into the run but I can definitely still run on it and is actually way way worse when I stop. Doesn't hurt at all when I run downhill. Hurts to walk on for the rest of the day after I run and when going up/down stairs and after sitting cross legged is stiff. Also has a grating and clicking sound when I bend it. Does this sound like itbs or is it something else?

I guess that may stand for ITB Syndrome? - but I'm not good on jargon! the Ilio Tibial Band runs down into knee and is very often over-tight, so affects the knee. I would check out a local Bowen or NST practitioner (I am just retiring as one in Malvern!) and try a couple of treatments. Can work like magic, but no guarantees. If you find one on-line check out all their qualifications, or welcome to ask me to assist. Other therapies could be equally beneficial. Good luck. Timbow

My suggestion would be stop running for a few days and gently stretch out your hips and IT band and think about where your running and starting to have pains does the road sloped on the same side as your hurt leg? Are your sneakers worn out? It could be a meniscus tear or tendon tear …. Honestly it could be lots of things but the simplest thing to do is take a break and stretch out your legs and hips and after a few days go out for a slow run and check the road slope Im sure it slopes to allow water run off and if does figure out how to avoid the sloped section and if this doesn’t help its time to go see the doctor for X-rays.
