Questionnaire Help Needed to assess Satisfaction With Chronic Pain Services and Treatments.

Hi all, I have at last come up with a questionnaire to find out what treatments, support and services chronic health sufferers are using and how helpful they have been. It is also an opportunity to vent and share your stories if you wish to add them.

The questionnaire is straight forward, but it is my first time so please be gentle with me!

There are always horror stories on here of poor services and support chronic pain sufferers receive and how disgusting some of the "care" is. My aim is to present this to Key people to show how we are suffering. I may be wrong and the care may be good for most but this would be a good way to measure how things are and how they could be improved if needed.

There is also very little discussed in the media about chronic pain which I am trying to get highlighted more. So the results of this could help.

It is completely anonymous so don't give names of Health Professionals, or hospitals please.
