HRD is mentioned a lot on this forum and Im wondering if its automatically tested for or if its different at different hospitals. Its never been mentioned to me and I forgot to ask last time I spoke to consultant.

Hi Linda Hope youre well and all recovered.HRD was mentioned to our oncologist (JW) and he didnt think it was relevant to my particular case as it wouldnt change my treatment. They have to send a sliver of your tumour sample to America to be tested but currently its paid for by the pharmacompany. That was my understanding anyway.
You could contact JW and ask him.
My blood work is all going the wrong way...CA125 and WCC going up markedly so something is going on. He has asked for an urgent CT scan for me. This has happened sinced I finished my 6 cycles of post mastectomy Nab Paclitaxol. It seems just the Avastin isnt keeping my Ovarian cancer stable currently.
Im pretty worried. My fat arm (lymphodeama) just adds to my joy...
How are you getting on? Whats your treatment now?

Hello we didnt manage to meet up in Bearsted did we ! Im starting Niraparib , should have started already but a bit of a mess up with paperwork etc.Sorry that things not too good with you. Hope they sort you out with a new treatment.
Im not under Dr Waters now, yet another change of consultant, on no.5 now!
I also attend Maidstone hospital, and asked my oncologist if I had been tested for HRD, and he said no.

If your tumour is positive for HRD, it means that certain treatments are more likely to be effective. One of those treatments are PARP inhibitors.
Thank you its clearer to me l think, its obviously linked to trials by the sound of it. Which because lm stable from my chemo 2 years ago, not had anything else. l think the more information we can have & options the better. Generally they test for HRD if you go on a trial and it’s relevant (the pharmaceutical company) pay for it. The reason is it’s expensive and not fool proof. I spoke to my onc about it. Sue
Testarossa71 posted a very good message about HRD. It’s worth looking up. NICE have only approved the drug protocol this year related to a negative BRCA and positive HRD test. I live in Scotland where we are still waiting approval so HRD testing was not mentioned at my diagnosis - v annoying. Sounds like you are having a tough time… so sorry about that. Homologous recombinant deficiency (HRD). I had my original tumor tested at Caris Labs in 2018. I’m not sure if it was tested for HRD, but the results showed other mutations that targeted therapies may work well . I learned that after chemo, DNA can be further mutated, so my Oncologist ordered testing through Perthera Labs. I was actually pleased that the results found HRD. This means that the Parp( Olaparib) may work better. I am BRCA negative, so no genomic , but HRD positive, which is considered a somatic mutation. I heard that 20% of women with ovarian cancer have this mutation. I am entering the Capri trial soon, so guess what?!? Another biopsy😳! This one will be sent to a different lab. It will be interesting to see if the results are equivalent.

Thank you very much for your post. HRD testing is sometimes included for those who are newly diagnosed with ovarian cancer. If you are not newly diagnosed, you can speak to your clinical team who may be able to advise you.

So does it mean that people without the HRD and BRCA negative may not benefit from a PARP?

Without BRCA gene changes and HRD you can still benefit from PARP. This is why Niraparib and Rucaparib are available for those without these gene changes. There is evidence for a good response to PARPs for those with BRCA changes and HRD and this is what informs the NICE decisions which is why Olaparib is currently available for this group under certain circumstances.
