Help with my mri result and my pain

Hi there, good afternoone, evening, whenever and whatever you are 🌻,

my name is kasra and it is second time posting in this community, actually iam suffering from sciatic pain for 1 year and these days it growing to my right leg and medicines like naproxen or celcoxib and etc cant help anymore, also stretching exercises wont help, i have strong back muscles and flexible body i dont think exercise help me more, but pain is going unbeliveable worse like: feel burning in hips and weakness in quadriceps muscle and resting cant help,and mri result is normal, i also do an x-ray but nothing unusall l, they say everything is right but my facet joints are suspicious but my back is still flexible and no pain, please take a look at mri result bellow:

MRI of lumbosacral:

The lumbar spine shows a smooth lordotic curve with a normal promontory angle. The bony spinal canal displays normal width. The vertebral bodies are normal in their number. the intervertebral disk spaces are of normal height, and the disks do not project past the posterior sufcce of the vertebral bodies is any segment The conus medullaris terminates normally position at the Li and divides into its filaments. the dural tube appears normal in its lumbar portion and evaluable sacral portion. he bone mrrow of the vertebral bodies appear normal. he imaged soft tissue show no abnormalities.

Conclusion=normal

actually xray shows no abnormalities, it is only about endless pain which makes me diewishing for now, cuz i need to sit on chair for long time(my work), and then pain become extermly bad, and may lose my work, any suggestion what kind of curse is it ?

and all the thing doctor said me is you need to wait and will be right, also my BMI is normal, and no other problem in my body

Hello and welcome. Sounds like you are suffering quite a lot. Luckily you've had quite a few investigations done because it's always good to rule a few things out. Your mri results do show a very healthy spine which is a good sign. I would recommend having your sacroiliac joint checked. A sacroiliac joint irritation /dysfunction can be very debilitating. Also you mention that you are very flexible. You might suffer from sacroiliac joint hypermobility which is difficult to treat and not many GPs or orthopaedic consultants check for that. It wouldn't show up on an xray or mri. But this obviously is only my first idea that springs to my mind. I hope you get help soon

Thanks for the replay, yes you are right it may sacroiliac irriation, but my doctor never checked it, i need to find a new good doctor and ask him about this, i remember the sciatic started suddenly, one night after come back from work i had been go to gym and do heavy squat and legpress, and then come back home and fall at sleep then tommorow wake up with bad sciatic pain, in this black year i lost 8kg weight of my legs and also sweats in bed everynight

Definitely get it checked. Sacroiliac joint problems are often overlooked. I also wanted to add that sacroiliac joint problems are often associated with a tight piriformis muscle. The piriformis muscles runs close to the sciatic nerve and can mimic true sciatica. It's called a piriformis syndrome. That might fit your pain as well. Because you have to keep in mind that the mri didn't show any S1 nerve compression at level l5/s1 which basically rules out true sciatica

I have had sacroiliac problems in the past , i had an X-ray guided steroid injection which really helped that particular problem. I hope you are able to find out whats wrong and get the right treatment as pain wears you down. Best wishes 🌸

Hi, I have found somatic exercises very useful, I did the psoas stretch exercises and they really helped with pain in the SI joints. This lady on YouTube is good:
