The aim of the survey is to get an update from the patient perspective on their transition and how that can affect them and their disease. The survey closes on Sunday 2nd November currently although subject to change.

The survey is hosted by Qualtrics. A favourable ethics opinion has been given by the University of Surrey for this survey.
