My 81 year old wife has been diagnosed with Large B-Cell Lymphoma. Her first diagnosis was follicular NHL (2021) and her first treatment was 6 rounds of R-CHOP (July-December 2021) . Her petscan after 4 rounds looked very promising, but her petscan after the full 6 treatments indicated that her lymphoma had progressed to Large B-Cell stage III B. She is now 4 cycles into her 2nd line of treatment, Revlimid 25mg pill (21 of 28 days) and Monjuvi infusions (now every 2 weeks). She is looking forward to a petscan in mid June to see if this course is effectively working. IF this 2nd line of treatment is not working, can she take an additional round of R-CHOP (maybe 8 courses instead of 6) or is one time on R-CHOP the only sensible treatment and that we should try something else? Suggestions or good experience?

Hello Ron I’m sorry to hear about your wife’s condition. I too had follicular NHL which transformed to a more aggressive large B cell stage 3/4. I too had R-CHOP initially but the CT scan after three rounds showed it was not working. So I then went onto RGDP chemo/immuno treatment and all this did was hold it but not improve it. I don’t know where you live Ron but I’m in the UK. I fortunate enough to then be referred to the Royal Marsden and because there was diffusion of the Large B Cells and because I was otherwise in generally good health I was able to go onto CAR-T Cell therapy. Miraculously 4 weeks after my infusion I was cancer free and I’m pleased to say I’m still in remission today, a year later. I don’t think your wife will be recommended R-CHOP again. My understanding is that other treatment options are used if you fail to respond to two different regimens of chemo. I hope this is helpful and wish your wife and you all the best in your efforts to get her well again.
Hey Ron. My wife had a similar FL to DLBCL progression. After 6 cycles of R-CHOP she had a 4 month PR. She got a 3.5 year CR with CAR-T. When that relapsed she got a 21 month (and counting) CR with an NIH ViPOR clinical trial.
