Persistent gout in renal transplant.

Hi,

As I understand it many of us will have had gout, I have for 30 years since my transplant. The last 3 years it has been relentless and the past few months: constant.

I have been given colchicine and allupurinol but it's not seeming to help any longer. The situation is quite serious as I'm unable to exercise properly and that can have knock on issues.

My createnine is normally above 300 so the kidney is not at its best but not bad enough to put me on the list for another transplant.

Has anyone else been in this predicament? Found a solution?

For what it is worth, my Dad has gout. (I'm the transplant recipient. ) He takes celery seed daily, bought in bulk from Amazon, and it does pretty well for his gout. That's just one data point. His body isn't a soup of medication like mine is. Consult your doctor, please.

The issue with gout is high uric acid in the blood. Red meat, and other food high in purines will make gout worse. I have Psoriasis and Psoriatic arthritis. It emulates gout. My uric acid level was very high. I passed multiple kidney stones formed of uric acid. They finally put me on allopurinol and also sodium citrate. That is a solution I drank in hibiscus tea. Check you bi carb level in you blood, often the Co2 lab. The doctors should be doing all they can to help you becaus it is painful but also NOT good for the kidney.

To be honest, i had it years ago and one time was all it took for me. I was on allopurinal but they say it is not the best for transplant patients. For me, the best answer was a change in diet and raising my water intake drastically so the body has plenty of fluid to flush thru the system. I also cut back on my protein intake. Red meat including pork and alcohol are bad culprits for flaring gout. Also fatty foods like gravies and things. I have heard many people also use magnesium to help keep the uric acid build up at bay. And i had one dr tell me that drinking a coke a day can help it. I wish you the best.

Hello Ikni - Though I did need to resort to alopurinol, I do also eat celery every day! I know that might sound weird, but a naturopathic doctor I spoke with suggested I try it. I do some online there may be some good rationales for this. I also eat a near vegan diet. Not sure if either of those thing have made a difference, but maybe worth a try? Either way, best of luck to you.

Dear lkni!

I'm sorry to hear you're suffering from gout.

For me similar patient like you.

- Drinking plenty of water (3 liters or more daily), which is excellent for overall health and may help with my gout. My uric acid level is reduced from around 8 to below 7 after 6 months,

- I don't eat meat much, relying on milk other veg for nutrition. Milk helps reduce the uric acid.

- Avoiding fructose sugar, as it can contribute to gout.

See your nephrologist to discuss if switching to Febuxostat could be beneficial for your specific case. Febuxostat seems may be more suitable for a person with degraded kidney function.

Here's some information about celery seed that may be helpful:

-Celery seed contains compounds that might reduce uric acid levels and inflammation, potentially easing gout symptoms. However, more research is needed.

I have not tried it yet.

I have had it on my right big toe a couple of times. My Podiatrist told me that next time it happens to come in and he would give me a nerve block.

Hi I have gout for many years, I had 2 transplant in 1989at age19 I had gout they put me on allopurinol. I have been taking that since maybe June 89, that was my second transplant lasted 13 years. Then chronic reaction. Was the reason for failure. Then I had 3rd transplant that lasted about 17. Still taking allopurinol. I've never had any more episodes of gout like uric acid is not as low as it should be but I haven't had a flare up.

Hi,

Sorry to hear about the gout. I think it's one of the worst pains to have. I have it on and off. I will second what most people are saying. While we now understand that diet can have an impact to gout, genetics and medications are also a big culprit. My gout attacks are related to lasix which can increase uric acid levels leading to gout flares, especially if you don't drink enough water. Celery seed works well, but I am partial to dried cherries. They have some of the same compounds that celery has to prevent the formation of uric acid crystals. There was a study done where they found people eating only 5 cherries a day prevent gout flares. Eating less protein and dairy along with more fresh fruits and veggies is also a huge help. - basically, the diet we are all supposed to eat, but rarely do.
