You would have to contact (or ask your doctor to refer you to) a hospital that performs kidney transplants in your area. If you are in the U.S., here is a link to find transplant centers in your state: kidney.org/atoz/content/how...
The transplant coordinator at the transplant center will guide you through all the testing you need! You can learn more about the transplant evaluation here: kidney.org/atoz/content/kid...
Communication is essential with any medical situation. I have always found it helpful to write down all my questions I have for my medical team. Next when I go to my appointments, I take someone with me so they can write down exactly what the doctors and team members tell me. It makes things a lot easier for one person to ask the questions and listen to the answers while the other takes notes. Also--make two copies of your questions--one for your doctors and one for you. That will insure nothing is missed. Try this--I would bet you will make better progress in your care and treatment plan.
