CKD or old age

I am 73 years old and my gfr is 52. It has been in the high 50's apparently for a few years. At my last doctor visit 2 months ago my doctor's PA told me i have ckd. My creatinine is 1.1 and all other labs are normal.

I do not have high blood pressure or diabetes or cancer. My doctor insists I do not have ckd, just old age. I am due to return in January for another blood test.

The only med I take is for cholesterol which is at 185.

Do you have any thoughts? It is making me crazy.

It sounds like you are ckd stage 3a since you have been below 60 for a few years. The decline could be from age. Just being dehydrated could lower your GFR temporarily. You are at an early stage. CKD is a slowly progressing disease. When I was at that stage, my doctor did bloodwork every 6 months. I wouldn’t be overly concerned at this point. Make sure you are well hydrated before your next blood test. Good luck to you.

There are some reports on this including physiological kidney changes with age, suggestions of over-diagnosis of CKD because of the age factor etc. I can send any of these, if you wish.

My mum is aged 69 and been considered stage 3 for 14y. Her CKD is more complicated as she only has 1 kidney following a nephrectomy >7y ago after kidney cancer. But she is doing well in terms of most markers etc 🤞🤞and takes no medication but follows Nutritional Therapy.

I presume your total cholesterol figure is after the medication, as it's normal? Mum's is significantly higher and >normal which we is likely due to her CKD and/or post-menopause.

Thank you for your response. I actually have tons of literature on overdiagnojsis of ckd in elderly, based on gfr. My cholesterol is 185 I take simvistatin. I have exercised 3-4 times a week for 20 years and continue this with moderate weightlifting class 3 times per week and Zumba on Saturday. I am not overweight at 150 pounds. No diabetes or blood pressure.

I have been on statins for 20 years. When I first started the level was 218.

Thanks. I'm guessing that was just ~12% over range? Maybe raised due to menopause or possibly CKD onset (despite not being diagnosed).

Yes, in range now whilst on statins. I meant the small over range at 218, before statins.

Thank you. As I recall, everyone in my family has high cholesterol on my mother's side. Most refuse to take treatment. Yikes!

Plenty don't take statins now. The hypothesis of cholesterol causing cvd is outdated and questioned, with good scientific evidence.

Here is a link to explanation of eGFR calculation that might be helpful. I am older and diagnosed with CKD level 3b at age 76. I was put on a CKD diet which has helped my eGFR stabilize. Age is a factor in calculation of eGFR. kidney.org/kidney-topics/es...

I agree with your doctor except that I'd avoid drugs like NSAIDs that are harmful to kidneys, also red meat. You don't seem to have the co-factors like high BP, proteinuria or diabetes that make progression of kidney disease more likely. I am 72 and have been in stage 3 for years, though my blood work is quite whacky in many ways, the eGFR only bounces up and down by a few points.

Though I have to say my eGFR has been as low as the 30s before I went on a kidney friendly diet which was also good for lupus, which is in remission, and heart arrhythmias, which are rare now, and lost weight which helps with energy and BP.

Thank you. I've stopped aspirin and I have never been a red meat eater. I've upped my water to 24 ounces per day. I dont have BP or diabetes and I can't find my urinalysis results. I have always exercised a lot for 20 years or more. I go back in about 30 days for another blood test and maybe urinalysis. I feel kind of scared, but we'll see what happens. Did you have proteinuria?

The reading and search I have done indicates that most or many elderly have age related ckd which doesn’t appear to progress beyond Stage 3ish. But good to do all ckd healthy lifestyle exercise weight management etc to keep well and stable in any event. Ckd appears to be quickly and over diagnosed which causes a lot of needless anxiety. Keep monitoring but maybe a CT scan can show the health of your kidneys. Mine showed healthy kidneys and function so I stopped worrying.

Thank you for your reply. I go back for a retest at the end of January. My diet is full of fruit and vegetables and I exercise regularly 4 times per week for 20 years. The worry is the worst thing, but we will see. My weight is good. I'm glad you dont have to worry!
