Much of it depends, however, on what is the underlying reason for your situation - it can be autoimmune conditions, diabetes, high blood pressure, infections, polycystic disease, overuse products like aspirin, etc. So that drives approach and treatments. In my husband's case, T2D took his native kidneys. You mention genetics as being responsible for your situation - others here may share your particular genetic problem. There is a kidney friendly diet, basically a low protein form of the Mediterranean diet, if you want to go all out for your kidney health. Many people on this forum have been at stage 3B for years with just these guidelines, but it depends on whether or not the process that caused your CKD is still active.
