I am 42 years old, 177cm tall, weight 80 kg, last year I found out about kidney dysfunction, my egfr fluctuates between 65-55, I am in contact with a nephrologist, I do not feel any discomfort, all other tests except creatinine are normal. I love to do sports, especially cycling, gym, boxing. I could train every day, while the nephrologist categorically forbids intense efforts. I tried to look for other doctors to compare opinions. Unfortunately, I have not been able to determine anything concrete. The opinions of others say not to strain too much. Does anyone have a similar experience? Can anyone suggest where the limit is? How to determine the training intensity?

Hi,
If you want to save kidney function listen to your Dr. You can prove/disprove their instructions quite easily. Take one blood test after no activity, then take the next one after strenuous activity, the answer will be clear to you.
Tip: try to have bloods done the same time of day after doing the same things each time. This will give a more consistent result and something to better track you kidney function.
Youre correct that data seem limited. Maybe unsurprisingly the key highlights in the paper linked below (authored by exercise-related organisations) are the positive effects of exercise whereas kidney organisations/medics seem more cautious, as normal.
Personally I would research/see a qualified Nutritional Therapist that specialises in CKD who may have experience with nutrition and lifestyle for previous clients as well as knowledge of current research. Medics eg nephrologists, do not have the relevant qualifications or experience in these areas.
in May 2021, I did preventive tests, all came out ok, except for creatinine, which was 1.4, I ignored this considering that exceeding the norm by 0.2 is nothing bad with quite intense exercise and large amounts of protein in the diet (about 3 grams per kg of body weight !!!), I did not consult these tests with any doctor, the situation continued until September last year. In August 2022 I had an accident, I was hospitalized, I suffered muscle breakdown (Rhabdomyolysis), by definition it is a killer for the kidneys. At the worst moment, creatinine was at 3.8. Flushed the kidneys with a lot of fluids (drinking, drip)Six months I took a break from sports, watched the body. Results at the level of 1.2 -1.45, I slowly returned to sports, the results are the same, radical changes in diet I did not do, I pay attention to limit protein.
