3 days ago a had an appt with a neph. I had 0.27 gr/24h protein, 7.6 mg/dl serum acid uric and some elevated cholesterol levels. She said "you have nothing to worry about. watch your diet. it is enough" I was not satisfied with her diagnostic because my protein and serum uric acid levels were somehow worrisome.
So, I made another appt with another neph. She said "yes, you have something which is to be cured." I was prescribed a med for high uric acid. She also changed my diabetes pill. She also prescribed meds for cholesterol. She said it is trade off, either your kidneys or extra pills which usually nobody agrees. Later I was interested and learnt that proteinuria could be due to high uric acid levels.
I think attitude of the next neph to resolve the problem was smart. Now I feel more satisfied with the treatment method of the last doc
So, if your neph says "nothing to worry", be careful! Seek the true diagnostic and treatment. Educate yourself before it gets too late

I was diagnosed at CKD 3b. I too had high uric acid and prescribed a pill and also a pill for high cholesterol. That was 5 years ago and is working for me. Keep us posted on your new Doctor.
I too had a nephrologist who kept saying that I was stable though the creative levels kept increasing with time until one day he said you are stage 4. He didn’t give me any significant advice on diet etc except to not be on a high protein diet. I changed my nephrologist. The new one is a little better at explaining things but he also did not give any dietary recommendations. In fact said I should not cut proteins too much. Medication wise I am on low dose of olmesartan, cholesterol and Calcitrol and take allopurinol for lowering urea and preventing gout attacks.
Hi,Do you mind sharing the name of the medication prescribed for your uric acid? I also have protein in my urine, but my nephrologist hasn't yet given me anything on how to treat it. I see her in January although my eGFR did improve from 30 when I was in the hospital to 47 then to 44 I sometimes wonder about getting a second opinion. I was seeing her every three months, but changed it to 5 months since all my lab work had improved. I'm on Metoprolol 25mg, Sodium Bicarbonate three times a day, and iron pills, but hasn't spoken to me to see a dietian. Will be freaking out when I see her next month, and she requests new labs, glad your feeling much relieved after seeing a second Dr.
I'll jump on about the uric acid meds. I was prescribed 100 MG of allopurinol twice a day. I had one attack and knew i never wanted another one! I've been taking this medication for a few years and never had another attack. My uric acid level stands around 5.7Hope this helps.
Hi, Thank you odd because I on was on Allopurinol back in May when I was admitted to the nearest hospital by me. Never had an an active flare up, but it showed up in my blood work. In June when I seen my nephrologist going over my medications she wondered why I was given Allopurinol, even though I do suffer from acid reflux, but never an episode of Gout. So my nephrologist stopped it.
You were probably put on allopurinol because of high uric acid levels showing up in your blood work. I'm not sure what the protocol is, but keeping that level at certain number (not exactly sure what it is right now) almost guarantees that you won't have an attack. And, if you ever had one, you don't want another. Also uric acid crystals form in your joints when blood levels are high and can eventually ruin a joint.. I had an Xray once when I thought I broke a toe and was shocked to see those crydtals around my toe joint and I only had one flare up.hope this helps.
Hi Dr mind,I never had an attack thankfully I have read about it online to try and educate myself. I see my nephrologist next month, and see how I'm doing she usually orders bloodwork to see how I'm doing. Hopefully you will never get another flare up thank you for your reply.
Thank you it's just weird because at one point at one hospital they gave me Allopurinol, then when I got to see my nephrologist she took me off of them. She said if I didn't have an flare up, like joint pain or other signs of gout then for now I shouldn't take them. I'm guessing during some time and doing lab work my levels were okay. Not sure
Words of wisdom! I fired my first nephrologist, as my gift went from 61 to 36 under his care. I know it wasn’t entirely his fault, but he was absolutely useless when it came to being proactive and educational. All he basically did was read my numbers, tell me my gfr’, and told me to ‘drink a glass of water first thing in the morning.’. My new nephrologist is very proactive and seems to genuinely care about my health. So glad I made the decision to change nephrologists.
