I have normal kidney function except proteinuria. My doc just prescribed jardience is this a gamble or is it effective im afraid of new med
And I have ABnormal kidney function with NO proteinuria and my doctor refuses to prescribe Jardience.
Good you have been able to reduce a1 under 7. Assume you have diabetes? Jardience is not the point. What's important is proteinuria. Nothing to be scared. On the other hand you have not to underestimate. Not always but sometimes proteinuria is early signal of kidney problem. Even when all other blood elements are ok. Fix a new set in 3 month possibly with 24h urine test. If proteinuria will persist look for a nephrologist. Rgs
There have been several studies and research trials into the Cardiorenal protective benefits of SGLT2s and the safety of this promising drug. I’ll link a few at the bottom if you want to read them yourself. Jardiance has been shown in two studies by the manufacturer that focused on having almost the trial group with CKD in fact it is the only SGLT2 that was tested on ANY participants with CKD. The initial study was closed early due to success and the second study just completed in April of this year. In the most recent study the average eGFR rate was 37 and almost 500 participants had CKD 4 with eGFR of 20-30. Benefits, it (after an initial eGFR drop of average 5 points…I’ll address that last) raised overall eGFR up to 30% reduced serum protein 50% and reduced creatinine 50%, lowered the chance of a significant CV event by 50% lowered blood pressure 10% and decrease fatigue in CKD patients. The only thing this drug seems to struggle with is lowering blood sugar…for which is was initially approved. One of the initial detractors was the initial lowering of eGFR however this has been the subject of at least 5/6 studies I have read personally. They all conclude even if you are at an eGFR of 20 and initially taking Jardiance lowers it to say 10 as some reductions of 10 point were seen, their is no adverse affect of this as it protects the kidney functions so well and after about 6 week it recovers with no lasting effects. Also Jardiance is the only SGLT2 that has shown benefits to T2D patients AND folks like me with CKD but without T2D. However only Farxia is approved specifically by the FDA to treat CKD.
In 2018 there was some evidence the SOME SGLT2 class meds cause AKI in patient with below 60 eGFR. This is not an issue with Jardiance as the group that took the placebo had a higher occurrence of AKI then the group taking Jardiance.
So I just long-windedly said to you that all should be fine with your kidneys. Doc should want to monitor that but he should do that anyway if a T2D patient. Finally it should take care of you A1C as well.

Sunbear, that is an impressive drop in A1C and that will have great benefits for your health. Since diabetes is one of the causes of CKD and will make CKD worse, I would think that anything that makes the diabetes better is a good thing. How did your GFR react to the jardiance?
