Kidney CT Scan to understand

Hi

Please can you help me to understand my CT scan? My doctor told me that my right kidney is not working and cannot be treated. I went for a CT scan and here is the content.

Plain axial images were obtained with reconstructive coronal images.

Clinical indication: - Bilateral flank pain and burning micturation since 2 week.

gbl kidney is maJrototed with lateral orientAtion of renal pelvis seen. Right kidney is enlar&ed with a bipo\ar duuncter me ures 190 mm. Mnrked ballooning of renal pelvis seen with abrupt transition 10 a normal caliber

urre r - Cortical atrophy also seen witJ1 a maximu m cortical thickness seen at upper pole measuring \ 0. 0 m.m

An 1mpac1ed calculus is also seen at right renal pelvis, measuring 15.6 x 28.2 mm (TS x AP) witha craniocaudal extent of 23 mm (HU I 306). Findings are most likely due to Pelviureteric Junction Obstrucotin with if impacted calculus.

Left kidney is normal in size, shape and position.

No radio opaque calculus seen in left kidney, ureters ancl bladder. No mass seen in either kidney.

No evidence of hydronephrosis or hydroureter on left side. Bladder shows normal appearance without evidence of any mass. Nonhomogeneous fatty infiltration of liver parenchyma seen.

Appropiate bone windows demonstrate, degenerative changes seen in volving visualized spine. Reduced di space with vacuum phenomena seen at L5/S I level.

CONCLUSION

Right kidney is malrotated with lateral orientation of renal pelvis seen. Right kidney is enlarged with a bipo diameter measures 190 mm. Marked ballooning of renal pelvis seen with abrupt transition to a normal cali ureter. Cortical atrophy also seen with a maximum cortical thickness seen at upper pole measuring 10.0 m An impacted calculus is also see n at right renal pelvis, measuring 15.6 x 28.2 mm (TS x AP) with cran.iocaudal extent of 23 mm (HU 1306). Findings are most likely due to Pelviureteric Junction Obstruct with a'impacted calculus.

Talk with your Urologist. Kidney not working may or may not be an oversimplification. If anything ask for a 2nd opinion from another Urologist, not the Internet. We can't diagnose you.

So what did your doctor say your treatment plan is? Are they going to take the non fuctunting kidney out? I would think that letting it stay in you shriveling would cause some toxcity.

Non-functioning kidneys are not usually removed unless it is cancerous or has become huge due to polycystic kidney disease.I have had a kidney transplant and my native kidneys have continued to shrink in size to about the size of a golf ball. Generally, they do not remove them as they can continue to work, even slightly after a transplant. And no they do cause toxicity. So, I have 3 kidneys.

What WYOAnne said. I am getting my kidney removed next week after a "failed" pyeloplasty. That's what the nephrologist told me, that they usually leave it in place and that the kidney will scar and not present risks. AFAIK the main reason why they are removing it is because I still have flank pain and I never got back to a normal level of activity at my stage. There are likely other reasons and it was my Urologist's call to get it removed.

You need to ask your ordering doctor to explain the results to you and not just that your R kidney is not working. We are not medical experts on this site but can only speak from our own experiences. I would hate for someone to tell you something regarding your CT and be wrong.You need to be your own advocate and have your doctor explain. I would want to know the results without the medical jargon. I would want to know exactly why my R kidney is not working and is my L kidney OK? Or will my L kidney also become affected in the future.

That being said, there are many people that live well with only one kidney. I have a friend that was born with only one kidney. Those of us that have had a transplant, we only have one functioning kidney, our transpant.

ijaffar...make your doctor explain everything to you in detail! Sorry to say, it's the patient's responsibility to ask questions and (nicely) demand understandable explanations. If your current nephro doesn't do that, seek one who will. It's your kidneys, your life. Don't allow them to rush you out of an appt. Take a copy of your report with you and make notes on questions you need answers to, and go through it step-by-step with your dr. Once they see you're actually interested in knowing details and facts, they'll appreciate it and know that they're dealing with an informed patient...or at least one that wants to be better informed. You said it all! We all have to be our own Advocate!! You are so right....it is our health, our LIFE we are talking about !My nephrologist even plans for more time with me because he knows I will have questions. He explains too, until I really understand his answer. I write concerns or questions on a piece of paper or on my phone so I am prepared for my appt. I am already doing that now, for an appt. in November.

lol...my nephro & PCP allow more time for me too because I go into details and want answers to my questions! Glad you've got good docs. So important in our health journey. I wish people would understand that it's their right to receive good healthcare, and if they aren't getting it from their current medical team they need to find new ones. There's nothing wrong with getting 2nd opinions, or making appts with new docs to "interview" them before changing to their practice. If you don't get a good vibe from them during an initial consult, you're at the wrong place. The kidney stone is stopping the rt kidney from working, it says “obstruction “. You need a urologist, maybe have the stone removed. But definitely seek urologist opinion.
