I would want to speak to MPN consultant. Most GP s have little knowledge of MPNs .So would find it hard to assess possible outcomes of taking along with. Mounjaro.
Concur that this is a question best asked of a MPN Specialist who is familiar with hydroxyurea and the other meds used to treat MPNs. A GP isd unlikely to be aware of the treatment needs for someone with a MPN; however, if the GP checks for interactions the doc will see this warning.
tirzepatide in Mounjaro caution advised w/ oral drugs, esp. narrow therapeutic index; tirzepatide delays gastric emptying, may alter absorption of concomitant oral drugs epocrates.com/online/intera...
Hydroxyurea is a "Highly toxic drug with a low therapeutic index." drugs.com/monograph/hydroxy...
While this does not mean that HU and Mounjaro cannot be used together, but it does mean that caution is indicated. The risk/benefit evaluation is altered by this information. Would the intended health benefit of adding Mounjaro justify the intrinsic risk of a potential drug interaction?
Please let us know what you learn as other will benefit.
My husband was prescribed a different diabetic control injection med. but with similar appetite suppressing side effects as mounjaro. Having seen the irregularity of those side effects and the impact on his appetite, sometimes hungry sometimes not, I can’t help but think that using mounjaro would impact on your body’s absorption of the hydroxy. I certainly wouldn’t mix the two.
I’m sure a Mediterranean diet would be a better choice whether for diabetes control or weight loss, both of which your GP should be able to help with.
I asked this same question of my specialist nurse last week & she wasn’t aware of any reason why it couldn’t be taken with hydroxy, in my case.
I’d try to speak to your CNS or Consultant to be sure
please do not rely on information from a GP or nurse specialist. You need A MPN to make those decisions. These are not decisions either should be making.
Hi all. I have been on Wegovy (same) for about the same amount of time, started both about two years ago. None of my doctors have showed any reluctance to me taking it. I’m doing very well. Going on a diet is not at all like taking Ozempic or the others that are out there like mounjaro. It affects the brain chemistry and sates the appetite. The combination hasn’t harmed me. I’m taking 5mg Hu 5 days a week. The doctor started me out on the smallest dose of trizepatide (injection not pill form) and slowly brought it up. I am obese and have lost 67 lbs in two years. This has helped my heart (open heart surgery). Osteo and Rheumatoid arthritis immensely. I can now train three times a week also, which was impossible a few months ago and when I lose 30 more pounds I will be a good weight for my height.
I believe my health problems stemmed from my weight gain probably ET too. So I feel very fortunate to have been put on this drug and by the way, studies are starting to show it’s having a good effect on the heart. Personally my cholesterol markers are all normal, triglycerides 0.87, AC1 - 4.8, and glucose fasting 5. All of these were poor when I started except glucose. I am not a diabetic so am paying dearly for it. It costs $400.00 a month in Canada.
Hope this information has been helpful and best of luck with your research to use or not. I have to admit it has changed my over all health and life style in a positive way. My playlets too are in the normal range, which of course has to do with Hu and the good care I’m getting from my hematologist.

Big thanks for your reply…there is certainly much to consider.
My understanding is that , among other benefits, mounjaro significantly reduces inflammation. I have a friend (age 65 and not on HU) who tells me it has completely eradicated her arthritic pain in legs/feet that she has had for years and has also eliminated some ankle swelling. Amazing.
Where do you live? I am in Australia. Mounjaro has just been approved for obesity so can be prescribed for that purpose. However, it is not yet financially subsidised so will cost around $400/month. Am hoping it will be subsidised within the next few months- fingers crossed!
Oh I hadn’t known it was so good for inflammation also. My rheumatologist takes care of that and I’ve been on methotrexate and a few other medications for it and they work well for me.
That’s so great that Australia has approved it for obesity. Think they’re finally figuring out obesity is the cause of so many ailments. I’m in Victoria Canada with fingers crossed it will happen here soon too.
I had confirmation from my haematology team that it was fine to use both
I am on HU 3 capsules a week (MWF) for PV. After some trial and error medication adjustments, iron infusions etc. my counts are well controlled and my energy level restored. My only problem was the rapid gain of about 12 pounds in 6 months. On my 5’2” frame, that’s a lot!
My excellent hematologist prescribed Zepbound for me, but it was unavailable at that time. My pharmacy compounds a sublingual semaglutide which I have been using for almost three months. With my intermittent timed fasting program which I was already using and this med, I am finally beginning to see some results.
I was going to have to pay anyway as I am not diabetic and although I am overweight my BMI is less than 30. The compounded preparation is less expensive than the injectable and as it seems to be working, I am not looking for the Zepbound at this time
I have had absolutely no problems with this.
Did you have the chance to discuss with your consultant?
After my CNS initially telling me she didn’t believe there are contraindications with Hydroxy and Mounjaro, when I looked to order some, the prescriber rightly wanted confirmation from the team that they are happy for me to take.
Consultant wants to do further research before commenting further
