Hi dear. Unfortunately, yes these results are concerning. There are 3 things that will usually be found in a blood smear when MF is present. Immature white blood cells, teardrop red blood cells, and nucleated red blood cells. Blasts are extremely immature WBCs that should not be present in the blood even with polycythemia. Your blood results did not note teardrop RBCs but poikilocytosis means different shaped red blood cells so they may be present. I do not see any notes about the nucleated red blood cells which I suppose is a good thing but the rest of the results are concerning. I hope your bmb goes well and I will be thinking of you today. Edited to add: In your bloodwork notes they did note dysplasia so it may be possible you get a diagnosis of MDS instead of MF. I'm not informed enough about MDS to say though. Or you may have entered blast phase polycythemia. Please try not to worry too much until you speak to your doctor though . I know it's easier said than done.
Teardrop and pencil cells had been mentioned in previous blood tests. Bmb all done. This time by a very experienced Dr. It hurt but nowhere near as much as the last one. I took paracetamol and codeine whilst waiting to go in so I'm convinced that extra bit of pain relief helped. Now that everything is wearing off I'm beginning to ache and am incredibly tired.Results on the 18th

In my experience, we can drive ourselves mad trying to interpret blood results. In your position, I would phone the specialist nurse to ask for clarification or hopefully the person doing your BMB will be able to help. Please don't just wait and worry.
As the others note your current results are concerning but they cannot be interpreted without more history and additional information from the BMB. We all understand how worrisome the uncertainty is. Suggest you contact your care team to review your concerns. Perhaps they can put a priority on getting your BMB results back.

as Otterfield has said it’s best to discuss this with the expert once they have the BMB. results. We can advise on the straight forward stuff on here but it’s the job of an expert to advise on the trickier aspects .I had a BMB recently and tried to understand what it read, I couldn’t understand it, I asked the expert expert at Mount Sinai if I should learn more about how to understand it and he said firmly no. One expert or pathologist can have a different view on the same counts or BMB. Try to put it aside in your head until you get in front of an expert with your BMB results and labs otherwise it’s mainly speculation. Wishing you good luck and hopefully some peace of mind in the meantime.
I'm glad your bmb went better this time. I understand what it is like to wait and not know. Focus on eating well, exercise when you can and getting good sleep. Do something nice for yourself every day. Those things are within your control. I will be thinking of you.

We are here to support. Hard to diagnose online w/o more info, however your BMB, other tests and the MD's experience will be important factors in the final outcome. Keep up the positive attitude.
