U have 10 minutes just explain 1 thing bithering u most will not bother even to taje blood oressure how we naintain our medical symptoms etc in first few visit drs should knowthe all symptoms and than try to rule out one by one tet diagnostick everything related to that illness by gooing through root cause of medical ailment but nk drs are greedy to make money out of u when ask receptionust 30 minute us maximum they can guve aappountment for i know in pat it used to me 45 minutes for ling consultation.

What you describe is what I call "15-minute Doctors." Systems of care try to fill as many slots as possible on the theory that 15 minutes is adequate for most patients most of the time. Unfortunately, with medically complex cases this is not always so. You are correct that when we know that we need more time it is important to intentionally schedule a longer appointment.

There is a strategy I use to make the appointment for focussed and cover more in a shorter period of time. I always go with a written agenda. It looks like this.

Treatment goals

Define for your doctor what your priorities are. Can be very specific at times.

Treatment Approach

Define what your approach to treatment will be. Make this specific to your treatment preferences. ALWAYs tell your providers all the interventions you use

Updates/Issues/Questions

Write out the symptoms and issues you are having and a list of your questions. Always include a holistic review of what is going on, particularly with specialists. Only give updates/changes to an existing provider. Do focus on what is relevant for this appointment.

Treatment

Review all treatment options – let the doc know what you are doing now and seek input on what else to do next. Ask about ALL options, not just the one the doc recommends. Ask the doc to explain why the recommended option is better than other choices. Ensure that it is your goals and preferences that drive the decision about your treatment. This includes your own risk tolerance and what risks you prefer to take.

I also go with a written list of all medical conditions and a list of my current medications/history of medication reactions.

Doctors read fast. I have them just read some things. I use the agenda to highlight the things we have to spend time discussing. My docs all like this approach.

Wishing you success in managing your care.

I'm sorry you are struggling so much, it must be very frustrating. Has your doctor discussed whether your symptoms are related to your MPN? Which MPN do you have?
