Hopefully others in the UK can answer the hospital-specific question. I would note that is is quite common to need to find a MPN Specialist outside of your base-hospital. This is, in fact, the norm in most places. This is true in healthcare systems here in the States too.
There is a process you can follow to see a MPN Specialist outside of your base hospital. Others in the UK can speak to how that works. I am thinking the closest MPN Specialists will be in London for you.

I don’t know for sure but I doubt Royal Surrey haematology dept has a MPN specialist, maybe a haematologist with an interest in mpns but that’s not the same. If you are being made to change haematologists then I’d grab the opportunity and ask for a referral to one with a MPN specialist and clinic.
When it comes to London travel I prefer using buses rather than the tube and it’s usually fine if travelling outside of busy hours. Guys hospital is next to London Bridge station, so no need for bus or underground if you can get there using overground services.
