This would be a question for both your hematologist and the physician who prescribed the drug.
A quick scan of the published side-effects does list elevated platelets in a few studies. However, the increase in platelet count they observed was much lower than your reported value of 650. Below is one example.

Amitriptyline (aka Elavil) is a tricyclic antidepressant also used to treat migraine and neuropathy. It can actually cause thrombocytopenia, decreased platelet count.

I think you are on the right track in trying to figure out how things link together. Migraine is in fact associated with MPNs like ET/PV. If your thrombocytosis is secondary, it would be possible that whatever is causing the migraines might also impact thrombopoiesis. The good news is that there are other options of you find the amitriptyline is not viable due to efficacy or adverse effects.

I have ET and was on amitriptyline for years (for migraines) my MPN specialist wasn’t concerned, I didn’t have an increase in my platelet count while on the med, and didn’t have any decrease once off. Best of luck.
I have Myelofibrosis and have taken 10 mg Amitriptyline daily for years, with no effect on my platelet count.
