Pre-clinical Expression of MPN?

Question for you all, I was catching up with my cardiologist yesterday as I had an MI (heart attack) 4 yrs ago. Back then we can see that my blood numbers were normal and he assumed at the time that the cause was a vasospasm (only 20-30% plaque was found during an angiogram). The question he posed was is it possible that the PV was already in me though perhaps at undetectable levels that could have actually been the culprit in creating a blood clot in my heart?After all, I had a stroke in December and my platelets were only in the 600s.
