Question about PMF

Hi there. I want to know that can someone with a blood picture like that of polycythemia vera have Primary Myelofibrosis. I know that PV may transform into Myelofibrosis over a long period but can it be PMF with high rbc count and hemoglobin initially. My Jak2 V16f is negative with normal EPO.

My understanding is that the MPNs exist on a continuum and there are not always distinct lines between them. Marrow fibrosis occurs gradually over time for people with PV, so you can have some fibrosis, but not enough to qualify for a MF diagnosis. If you are JAK2 negative, has the doc checked CALR and MPL? Hope you get some answers soon.
