Thanks Paul, and the beat goes on. I guess that we shall have updates (2008, 2016) as the research continues. It is an ever changing world, this one called MPNs. I hope that the next update is sooner than 8 years!

Hi Paul, This doesn't mention a diagnosis of pre-fibrotic MF separate to ET but the WHO does, is this not up to date?

I have read at least one article that said the British Classifications for MPN diagnoses do not include the Prefibrotic MF category separate to ET, which is at variance with the WHO MPN diagnosis categories, maybe that's why it isn't included? It is hard to distinguish even with bone marrow morphology according to my haem here in NZ, however my BMB has not shown any of the things associated with Prefibrotic MF so I'm happy with that. I think there has been at least one research that looked at groups of MPN patients diagnosed and classified using the British Classifications, they then re-classified them using the 2016 WHO rules and a fair number of the ET patients (I can't remember the % off the top of my head but it wasn't a small number) were re-classified as Prefibrotic MF. They said this was important as the diagnosis of Prefibrotic MF definitely has a poorer long term prognosis compared to ET, and would probably change the treatment plan for the patient, and not leave it to watch and wait for so long.

This is similar tool but gives progression risks for PV and ET, 10% and 4% respectively every 10 years
